2. Support Coordination (Coordination of Supports)
This intermediate level helps you develop the skills needed to understand and maximise your plan. A support coordinator will efficiently work closely with you to ensure a mix of supports is used to increase your capacity for independent living, build up relationships, manage service delivery, and engage in your community.
3. Specialist Support Coordination
This highest level of support is designed for participants with more complex needs. A specialist support coordinator provides additional assistance to manage challenges within your support environment and ensure consistent service delivery.
